  • This quick check helps you notice when that weight is starting to build — so you can step back, ease it off, and stay in control before it gets too heavy.
    Everyone carries a load — this just helps you see how heavy yours has gotten.

  • How to Do It: Read each statement and rate how often it’s been true for you lately:
    0 = Never 1 = Rarely 2 = Sometimes 3 = Often 4 = Very Often

  • How this helps: by determining your level of unmet needs, we can develop a plan to build your resources and resilience.

  • How you start: answer all of the questions in the Physiological Needs questionnaire.

  • PBYJCS will contact you within 8 days of completing any level to discuss your results and how you would like to move forward with us. If your responses fall below our critical level, we will be in touch sooner.
